The Dr. Dayton M. Formal Memorial Award recognizes outstanding leadership in the advancement of library and information services for Canadians with sight loss.
The award honours Dr. Dayton M. Forman, a humanitarian and longtime CNIB volunteer and advocate for the rights of Canadians with vision loss to have access printed material.
The award consists of a silver medal bearing the likeness of Dr. Forman, with an inscription in print and braille.
